May I propose that one of your girls undergo a thorough rectal examination. 
This could be the sequence:
1- The shy girl (much like Alice) walks into the doctor's office with the appropriate referral slip
2- The doctor takes the time to explain what is to be done and why, to gain her confidence and trust but the thought of having this stranger intervene up her private chute makes her resist somewhat.
3- Thus, the doctor finishes the preliminary routine and, noticing a slight bulge of her waistline, he would instruct her to completely undress and get on all fours on the examination table which she does with lots of angry prompting from the impatient doctor. 4-After the cursory visual examination of the exterior region of her genital area and the routine rectal temperature reading, he would then proceed with a deep digital probing of her vaginal and anal orifice to detect some form of blockage inside her rectal cavity in need of immediate attention.
5-To try to check blockage, doctor uses an extended Kelly analscope to see inside 
but meets with quite a bit of resistance. 6-He then administers a good two quart soapsuds cleansing enema using an extended enema nozzle.
7-After voiding herself, the patient would return to the table on all fours for the end of the examination. 8- For the final probing the doctor must use the three pronged rectal speculum much like he did wen he examined Rachael's vagina in her second examination sequence.For he rest I leave it up to your imagination.
